About Us
Charmaine CT Outreach, LLC provides Supportive Services to help you successfully transition back to the community.
My passion has always been helping people navigate life's most challenging transitions with dignity, compassion, and confidence.
As a Master Social Worker in New York State, I am committed to empowering individuals and families by providing personalized guidance, advocacy, and practical solutions when they need them most.
I earned my Bachelor's degree from Mercy University and my Master's in Social Work from New York University. Throughout my career in public service, I have worked with individuals facing complex personal, medical, and social challenges, helping them understand complicated systems, overcome barriers, and connect with the resources they need to move forward.
I founded this practice because I believe that everyone deserves an advocate—someone who will listen, guide, and stand beside them during times of uncertainty.
My practice serves two primary populations.

Older adults, individuals with disabilities, and their families as they navigate Medicare, Medicaid, long-term care planning, nursing home placement, hospital and rehabilitation discharges, home care services, durable medical equipment, and other complex healthcare and social service systems.

Individuals returning to the community after incarceration by helping them navigate the steps toward rebuilding their lives, including restoring professional licenses, identifying employment resources, and connecting with essential community services.
Whether you are rebuilding your future after a difficult chapter, caring for an aging parent, preparing for a hospital or nursing home discharge, or trying to understand government benefit programs.
I believe every person deserves to be treated with respect, compassion, and without judgment. My approach is client-centered, collaborative, and grounded in the belief that positive change is possible when people have the right support and someone who genuinely cares about their success. It would be my privilege to help you take the next step forward.

Our Services
We provide confidential services at your convenience and in the comfort of your own home if you choose to meet virtually. There are no additional fees for virtual services.
In-person services are available upon request.
